Q Assignment: Graduate Attestation of Employment Instructions Purpose: To comply with ABHES Accreditation Standards, we are required to collect this information from Pima Medical Institute graduates. For this assignment, you will submit two parts: Part 1: The Graduate Attestation of Employment, and Part 2: Your current job description. Note: If you are not currently employed, submit the Résumé/CV Creation Worksheet instead of the job description. To prepare for this assignment ? review the Graduate Attestation video ? review the Graduate Attestation reading topic To complete this assignment Part 1: Complete the Graduate Attestation of Employment form Follow the guidelines below: A. Ensure that the job title on the attestation form and the job title on your job description match exactly. B. Do not use acronyms. If you are a medical assistant, for example, spell out Medical Assistant instead of using MA. C. If you have a dual role, such as medical assistant/front office clerk, please submit the job description for at least one of your roles. We will also accept both descriptions. D. The address for your employer on the attestation must match the address on your job description. If you are a contract employee, list the third party you are employed by. E. For the start date on the attestation list month, day, and year. If you do not know the exact date, please enter your best guess. Graduate Attestation Assignment Instructions F. Part 2: Submit your job description This is part of the verification of placement process we are required to follow by ABHES. Job descriptions must be either • provided on a work letterhead, by email, with a signature from your boss, or • pulled from an official site such as Indeed.com. Note: If you are currently unemployed, that’s not a problem. Instead of submitting a job description, submit the Résumé/CV Creation Worksheet instead. Signature Requirements 1. Sign the form with a valid signature. Below are several options for valid signatures, with examples. You will not receive credit for this assignment if it is submitted with an invalid signature. a. Adobe Signatures with Time Stamp (Highly preferred) b. Printed form, hand-signed, and scanned Graduate Attestation Assignment Instructions c. Digitally drawn Submission Instructions You will upload TWO documents for this assignment: 1) Completed Graduate Attestation form, and 2) Job description OR Résumé/CV Creation Worksheet assignment. ?
